Brokerage Officer

**Main Purpose of Post**:
To provide and develop an effective and efficient Brokerage Service to support residents to access appropriate home care, home based respite and nursing/ residential placements. To work within an integrated service that sources services to better facilitate patient flows and enabling individuals to be supported in their choice of accommodation. To arrange the delivery of services in a person-centred, cost-effective manner that meets individuals’ intended outcomes as set out in their care and support plans.

To provide a single point of contact for the co-ordination and provision of services, liaising with care management teams, finance teams, internal and external providers including Powys Teaching Health Board (PTHB).

To manage a system to ensure best value for procuring care and support services in line with agreed processes and to ensure appropriate contractual arrangements are in place for a wide range of provider groups.
