As an expanding team, we have an exciting vacancy for a Primary Care Facilitator to work as part of the Primary Care Services Team within Digital Health and Care Wales.

As a Primary Care Facilitator, you will be responsible for the delivery and support of digital technologies to Primary Care Stakeholders.

We are looking for a motivated individual who is willing to work, learn, and actively contribute within a team environment, possessing excellent communication and organisation skills.

Experience in the delivery of digital technologies is desirable but not essential.

Due to the geography and spread of our service portfolio, you will be required to be flexible and be able to travel to various sites across Wales as required; this may on occasion, require overnight stays.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

We are looking to appoint an enthusiastic and motivated individual to join our expanding team to support the delivery of digital technologies within Primary Care across Wales.

As a Primary Care Facilitator, you will provide a ‘hands-on’ approach to delivering national digital initiatives across NHS Wales, supporting GP Contractors, Pharmacies, Prisons, Opticians and Dental services through engagement, implementation, coaching and mentoring and evaluation; working together to enhance citizen healthcare across Wales.

This position will require you to build strong relationships to account manage Primary Care Stakeholders across a broad range of digital products and services; to include advice and signposting of existing products and services. Supporting the implementation of new products and services the Primary Care Facilitator will use a range of coaching and mentoring techniques to enable Primary Care Stakeholders to utilise digital services efficiently and effectively.

Evidence of delivering customer services in a Primary Care setting, working on own initiative with a proactive and flexible approach to working is essential.
