The Patient and Family Support Team provide psychosocial care to patients, families, and their carers pre & post bereavement. They act as advocates for people with advanced life limiting illness and their family / carers, aiming to enable individuals to make informed choices to manage their health, wellbeing, and lives independently for as long as possible.
- Supporting the qualified Social Workers and other appropriate colleagues
- assisting in psychosocial assessments and assisting in developing patient plans
- promoting individual independence, wellbeing, choice and control by supporting a variety of models of service delivery
- escalating any safeguarding concerns ensuring the person is protected from harm
- helping to turn strategy into action
- collecting data as required

North London Hospice exists to enable the best of life, at the end of life, for everyone. We work together with our diverse community to provide palliative care and support, when and where it is needed most. We provide tailored, personalised care for more than 3,000 people a year who are living with a life-limiting illness in Barnet, Enfield, and Haringey. We also support their families, friends, and carers.
