**Service/Ward/Department Summary**:
Tameside & Glossop Access Team provides an access route into mental health services. The Access Team will triage referrals to determine the most suitable pathway to meet the needs of the patient. They assess, risk manage, and provide short term support for people experiencing mental health difficulties.

**Role Summary**:
To work in a Single Point of Entry/Access team screening all referrals into the service.

**Key Responsibilities of the Post**:
To work as part of the MDT within the Access Team to provide an effective and efficient response to referrals from GP’s and other Professionals.

To assess, risk manage and provide short term support for people experiencing mental health difficulties and appropriately refer on and signpost all patients referred who need assessment and/or follow up from mental health or other services.

To attend Professionals Meetings when required and to work in collaboration with other services to ensure effective communication and sharing of information

To represent mental health services within the Integrated Neighbourhood Teams and provide advice and guidance to the MDT

To participate in the overall development of the service ensuring that the principles of Clinical Governance underpin all developments.

It is expected that you will deliver care in accordance with all Pennine Care NHS Foundation Trust policies and procedures, the Mental Health Act and community care legislation.

**Key Requirements of the Person Specification**:
Applicants must possess RMN / RNMH / Social Worker / OT or equivalent. Completion of Preparation for Mentorship / Practice Teacher / Fieldwork Educator qualification, or willingness to complete, is essential together with significant post-registration experience.

We are a 9-5 Monday to Friday service with opportunity for overtime if wanted. The service offers flexible working opportunities and will provide the option of a variety of in base and remote working, hours can be negotiable.

The Team are continually seeking service development opportunities and welcomes an enthusiastic, motivated and caring Practitioner to join our busy and supportive team in Tameside & Glossop.

Referrals are accepted from GP’s or other professionals who are concerned that an individual is experiencing mental health difficulties. The Team offers a comprehensive mental health needs assessment for those individuals who may require secondary care services, and is able to offer short term support in respect of any identified mental health difficulties if indicated.

As a Senior Mental Health Practitioner within the Access Team, you will work in collaboration with the Integrated Neighbourhood Teams (INT's) across the Tameside and Glossop locality. The INT's are a multi-disciplinary Team (MDT) that meet regularly to actively case manage patients who present with multiple complex needs. Within the INT, you will complete mental health and risk assessments to identify patients needs and provide short term, low level interventions to prevent escalation and crisis.

The age range that the Access service offers assessment is 16-65.
