An exciting opportunity has arisen for an individual to join the West Essex Stroke and Neurology Rehabilitation Team as Rehab Coordinator.

The role, based in Epping, will allow for some virtual/home working over a 5-day working week to triage referrals and manage patients along the stroke and neuro pathways. They will also be responsible for supervising the stroke 6-month review clinic.

Applicants will be considered across a range of professions; including Physiotherapists, Occupational Therapists, Speech and Language Therapists and Nursing if you have stroke and neurology experience.

- Triaging all referrals coming into the West Essex ESD service and neuro-rehabilitation inpatient beds at St Margaret’s Hospital. The post holder will be supported by the Clinical Leads and band 7 therapists in coordinating referrals.
- Communicating with and supplying in-reach to various acute hospitals and external parties to discuss patient flow through the stroke and neurological pathways in West Essex.
- For feeding back to Clinical and Service Leads on progress of referrals, incoming patients and discuss upcoming discharges to allow for timely transfers in line with KPIs and service specification.
- To contact those in ESD within 24 hours of discharge from Acute Hospital
- For supervising the Stroke Six Month Review Clinic

1. To triage incoming referrals for the Early Supported Discharge Service and the inpatient neuro-rehabilitation beds at St Margaret’s
2. To develop links, and complete in-reach, with acute hospitals
3. To communicate with the bed managers regarding outcome of inpatient referrals
4. To contact all ESD patients within 24 hours of discharge
5. To monitor the pathway of current patients
6. To monitor and supervise Band 4 Associate Practitioner in their organisation and delivery of six-month review clinic.
7. To maintain clinical records as per Trust and professional guidelines
8. To continue to work clinically within professional standards and clinical guidelines, promote best practice, through active participation in continuing professional development (CPD) and to undertake all aspects of clinical duties as an autonomous practitioner
9. To support service development and audit participation
10. To update the team on incoming patients into ESD and Beech Ward to prepare them for assessments.
11. To escalate appropriate concerns or queries
12. To provide ESD patients with a comprehensive explanation of the ESD service and answer any queries that arise, being sensitive to their current presentation and concerns
13. To support patient’s family or carers with concerns and address any queries they may have.
14. To demonstrate good negotiation skills across a range of issues.
15. To deal with initial, verbal complaints sensitively, avoiding escalation where possible.
16. To ensure that Trust and professional policies are understood and observed and that procedures are followed.
17. To participate in, and contribute to, innovations in stroke and neurology pathways in West Essex.
18. To represent the team
19. To collect and provide data as required
20. To plan and implement training
21. To contribute to the development of team objectives or projects in liaison with peers.
22. To monitor stroke pathways and advise the service leads accordingly.
23. To advise the service leads on issues of service delivery including shortfall, service pressures, etc.
24. To be aware of other development of policies and standards in other areas of the team and contribute accordingly.
25. To use specialist knowledge to guide triage decision making in line with inclusion and exclusion criteria
26. A potential to transition to a 7-day service as per National Stroke guidelines and developments.
