**Location**: Partner Hospitals and in the Community with occasional visits to our East Molesey office.
**Hours**: 20 hours (Mon, Weds, Thurs)
**Salary**: £12,000-£12,500 per annum
We are seeking a compassionate and dedicated Family Support Worker to join our team. In this role, you will have the rewarding opportunity to deliver vital support services to families within our partner hospitals. With a focus on providing non-judgmental and compassionate care, you will work closely with families of all ages, ensuring their needs are met and offering recommendations for additional support. **Role overview**
- Deliver Momentum service to families who are under the care of East Surrey, Worthing and The Royal Alexandra children’s hospital Brighton, with visits to other hospitals as required (Royal Surrey Guildford) along with community support across there geographical catchment area.
- Provide compassionate and non-judgmental support to families of all ages within assigned caseload, reporting needs and recommendations for additional support.
- Collaborate within the Family Support Team, managing and reporting on caseload.
- Establish professional relationships with clinical colleagues and other third parties.
- Maintain regular contact with families via various channels, following Momentum guidelines.
- Monitor and track family needs using internal tools.
- Input and maintain accurate records in our database.
- Refer families to relevant therapies and services as needed.
- Manage family referrals within the allocated catchment area, providing ongoing support.
- Attend relevant training, meetings, and family support events.
- Support marketing and fundraising efforts by providing updates, photos, and case studies.
- Act as a professional representative of the charity.
- Report safeguarding concerns promptly.
- Other reasonable requests in line with requirements of the role.
**Person specification**
**Essential**
- Professional experience working directly with adults, young people or children and families, being responsive to their needs.
- Strong communication skills (both written and verbal).
- Ability to work independently, adapt to evolving service needs and manage resources effectively.
- Proficiency in using IT systems including Microsoft Office and databases.
- Collaborative and empathetic approach, working in a people-oriented and emotional environment.
- Possession of a valid UK driving license with access to a car.
- Reliable internet connection at home for effective remote working.
**Desirable**
- Experience of working in a charity (health or care)
- Demonstrated ability to work effectively within a team, including collaboration with relevant agencies such as NHS services and third sector organisations.
- Experience in bereavement or social work would be advantageous.
- Trained in safeguarding

**About Momentum Children’s Charity**
We support families across London, Surrey and Sussex whose children are facing cancer or a life-challenging condition. We’re there whatever the outcome, for as long as needed, so that no family has to cope alone.
The charity was founded 19 years ago by our current Chief Executive and has grown enormously over that time. We currently have partnerships with 10 hospitals who refer families to us for our support, which ranges from therapies and counselling, to respite holidays and special family experiences.
We’re on a mission to help even more families with seriously ill children to keep moving forward, and we’ll need brilliant and talented people to share our ambition and make it happen.
