we are looking for an Acute Response Practitioner qualified band 7 or a development opportunity for a band 6 on completion band 7 competencies.

We are looking for skilled and experienced practitioners to join our team to support our community crisis response and the development and delivery of Plymouths virtual ward programme. We are also keen to hear from applicants who want to develop their skills and engage in training to become an acute response practitioner within our team.

This in an exiting time to join the team as our Urgent Community Response agenda expands and the virtual ward initiative develops to help support people in crisis to remain in their homes. CCRT is a multi-disciplinary team responding to people who need support to prevent unnecessary hospital admission. We are keen to hear from you if you are a nurse, OT, Physiotherapist, Paramedic and either have the skills, or are keen to develop the skills, to work as an Acute Response Practitioner.

Training and development opportunities and support will be provided to successful applicants, in particular to complete the advanced assessment skills module (on completion post becomes band 7) and develop their competencies to support both patients and the wider team.
