An exciting opportunity has arisen for an enthusiastic and motivated Transition Worker to join the Adult Community Learning Disabilities Team (CLDT)Trafford, based in Sale.

The CLDT is a well-established and friendlymulti-disciplinary team and collocated with our social services colleagues. The team also links closely with our colleagues in Greater Manchester, as part of a wider Greater Manchester Network.

The post requires an enthusiasm and commitment to working with young people with learning disabilities and ensuring as smooth a transition from children’s health services into adult health services as possible. The post holder will be expected to liaise closely with young people, families, children’s services, schools and colleges.

Managing a caseload under supervision, the post holder will be expected to work autonomously and as part of the team to improve outcomes for young people with Learning Disabilities within their local community, whilst maintaining quality lives and ensuring people are free from harm.

Applicants should possess excellent interpersonal, communication and literacy skills and the ability to work with a range of professionals, agencies, families and carers in a wide range of settings. You will be able to demonstrate a real enthusiasm for young people.

The post holder will receive excellent support from the whole team and clinical supervision from one of the band 7 nurses. There will be opportunities for the post holder to link into Greater Manchester as part of the Transforming Care agenda.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.

New entrants to the NHS will commence on the minimum of the scale stated above.
