**About The Role**:
**Location**:Hertfordshire - Community based in Hertfordshire with some homeworking.

We are looking to welcome an approachable and flexible Dementia Adviser to work across the local community of Hertfordshire. The role will cover the whole county to meet the needs of the contractual requirements. This role will provide a great opportunity to support people affect by dementia by providing information, advice and guidance.

You will:

- Support people to maintain their independence, improving their sense of well-being, and help them feel in more control of their lives.
- Build relationships with a range of local contacts and network with health and care professionals.
- Provide support face-to-face, on the telephone and online.
- Deliver one to one services out in the community and working from home.

**About you**
- Passionate about putting people affected by dementia at the heart of everything you do.
- Empathetic and non-judgmental in your approach with a commitment to equal opportunity.
- Have an ability to assess client needs along with the understanding of the need for client confidentially.
- Possess great time management and interpersonal skills, with an understanding of dementia and the needs of those living with dementia and their carers.
- Have excellent communication skills as well as strong organisational skills.
- Have a sound knowledge of IT systems.
- Able to work independently and as part of a remote working team.
- Be able to travel independently across Hertfordshire.
