Support Worker

**Contract**:30 hours per week between Monday - Friday, permanent
**Salary**: £22,387.61 (Pro rata £17,910.09)
**Located at**: Cwm Taf Hubs
You will promote the independence and empowerment of people regarding their own mental health and wellbeing. This will be achieved by utilising the principles of Adferiad Recovery’s Wellbeing Programme to support a person to develop an ability to self-assess their current circumstances, identify their goals to help address their social care needs.
To maintain a welcoming and supportive environment to Adferiad Recoveries beneficiaries within the communities of Rhondda Cynon Taf and Merthyr Tydfil, where our hubs are based. Allowing the community, the opportunity to fully understand the organisation and service we are providing.
**Responsibilities**:
- To have commitment to and demonstrate an undertaking of equality of opportunity and its relevance to different sectors within the community.
- Must demonstrate empathy towards Adferiad Recovery’s beneficiaries.
- Complete relevant intake forms, assessments and recovery plans for everyone referred to the service.
- Develop successful engagement with services users based on foundations of trust and respect within a relationship built on clear professional boundaries.
- Work in partnership with relevant agencies where appropriate to provide additional support.
- Show empathy and understanding towards service users at risk and the barriers they may face.
- Maintain and continuously update knowledge of local service provisions and agencies to offer appropriate partnership working, signposting, and referrals.
- Manage workload, administration and diary commitments.
- Maintain accurate and up-to-date administration and caseload records, including accurate reports of all client interactions and discussions.
- Adhere to professional practice standards and legislations including confidentiality, safeguarding, equality, diversity, and inclusion policies.
- Ensuring implementation of Adferiad Equal Opportunities Policy alongside all other Adferiad policies and procedures.
- Work in accordance with the Mental Health (Wales) Measure and the Social Services & Wellbeing Act 2014 and provide information, advice and assistance if required.
- Adhering to and ensuring correct Health & Safety in the workplace.
- Recognise from information gathered and observations whether a service user can participate fully in support provided.
**Support**:
Supporting service users within the framework of the Adferiad Recovery Plan to address identified needs in the following areas:
- feeling secure at home.
- increasing independent living skills.
- personal finance, budgeting, and accessing benefits.
- mental and physical healthcare.
- access to and managing of individual’s property.
- building and maintaining relationships with families, friends, professionals.
- developing skills and interests.
- pursuing training/educational/employment goals and aspirations.
- pursuing leisure/recreational goals and interests.
- developing links with the local community-based resources and opportunities.
- signposting, joint work and referral on to other specialist agencies, intervening in crisis situation.
- recognition of and coping strategies in respect of risk, triggers and avoiding relapse.
- To enable clients to exercise informed choices about their lives where possible and to enhance their social skills and participate in the community.
- Support individuals through a tailored support programme with a view to move-on to community engagement, vocational opportunities using Adferiad’s Recovery Programme.
- Providing information, advice and guidance as appropriate and in a timely manner.
