This role is offered as a 12 month fixed term contract.

We are looking for someone to join the Bristol Rough Sleeping Service in the role of Hospital Discharge Coordinator where you will work across two main Bristol hospitals to provide advocacy, support and quality discharge interventions for people experiencing homelessness while attending hospital.

You will work with patients experiencing homelessness, hospital staff, local authorities, housing providers and other organisations to improve communication, understanding, discharge practices and access to services for this often excluded group, Key responsibilities in this role will include:

- Working with clients in hospitals to assess individual needs, finding suitable housing options and improvements to care whilst in hospital via strong advocacy.
- Overseeing case management and providing line management, motivation and supervision to Hospital Discharge Workers to ensure they can deliver a recovery focused approach and positive outcomes for clients.
- Building and maintaining productive and positive relationships with hospital teams, internal and external stakeholders to develop effective accommodation pathways for clients from hospital.
- Developing training opportunities for hospital staff around homelessness and the barriers that people experiencing homelessness may face.

**About you**

This exciting role will suit someone with some experience of working with health services to support people with complex health and social care needs.
- Excellent communication skills and the ability to build effective relationships with a variety of people and services to reach positive outcomes.
- Knowledge of the services and legislative environment relating to people experiencing homelessness.
- The ability to deal with complex behaviours in relation to people and the empathy to provide person centred support to people who may be in a difficult personal situation.
- Above all we are looking for inspirational, committed individuals who have a genuine desire to support people to rebuild their lives.
