Job Summary:
A reputable and patient-centered medical practice in Bolton is seeking an experienced and dedicated Practice Nurse to join their dynamic healthcare team. The successful candidate will play a crucial role in providing high-quality care to a diverse patient population.
Key Responsibilities:
Provide comprehensive nursing care, including chronic disease management, wound care, immunizations, and health screenings.
Conduct patient assessments, develop care plans, and support the delivery of patient-centered care.
Collaborate closely with GPs, healthcare assistants, and other members of the multidisciplinary team to ensure continuity of care.
Offer patient education and health promotion advice, empowering individuals to take control of their health and well-being.
Maintain accurate and up-to-date patient records, ensuring compliance with all relevant guidelines and protocols.
Qualifications and Experience:
Registered Nurse (RGN) with a valid NMC pin.
Previous experience in a primary care or community setting is highly desirable.
Proficient in chronic disease management, including diabetes, asthma, and COPD.
Strong communication skills and the ability to work effectively within a team.
Competence in using clinical systems and electronic patient records.
