Pay : £22.83 - £29.42 per hour

Southwark

Our Local Authority client in Southwark are seeking an Occupational Therapist to support their Adult Social Care Team.

The Local Authority are currently piloting a dedicated care home therapy service that focuses on supporting the discharge pathway from hospital to care homes in Southwark. This service will cover the 7 larger care homes as well as input into a few of the smaller care homes in the borough.

Responsibilities:

You will be required to help facilitate effective working practices in the care homes by providing coaching, advice and joint working.
Take a key role in the implementation and evaluation of this new care home service, in conjunction with partners from the NHS, primary care and social care.Requirements:

HCPC registered Occupational Therapist with at least one recognised OT qualification.
Significant experience of delivering high quality case management to older adults, specifically with dementia to live well through identifying and addressing care and therapy needs.
