Job Type: Full Time/Contract
The Role
The post-holder will be part of a adult social care Occupational Therapy Team which provides a health and social care service to the local community. In general Occupational Therapist Assistants will need to have experience working within adult social care for a minimum of 1 year and have experience assessing for minor equipment and adaptations such as level access shower and stairlifts. They will be expected to care coordinate a caseload of people with varied complexity of needs The post-holder plays a lead role in coordinating the care and support for users (and their carers/ families) pulling in the expertise of other professionals as required They will also be responsible for accurate and comprehensive inputting oft information for monitoring and planning purposes The post-holder will aim to provide an efficient and cost effective health and social care service to service users, carers and their families, in accordance with agreed threshold criteria and using appropriate resources of Social Services, Health, the Council, the community and other non-statutory organisations.
