What We Do

As a part of our service, we provide comprehensive support to families, ensuring that clinical tasks are carried out safely in a home setting, reducing the need for hospital visits. We are passionate about providing individualized care for every child, ensuring their needs are met through effective care planning and collaboration with healthcare professionals.

Our Core Team Offers Help With:
- Performing clinical tasks at home, such as blood taking, specimen collection, and sleep studies.
- Collaborating with multi-professional teams to meet each child’s specific nursing needs.
- Training families and carers in nursing tasks.
- Discharge planning to ensure a smooth transition from hospital to home.
- Palliative care and end-of-life planning when necessary.
- Bereavement support for families.
- Supporting the transition of care from pediatric to adult services.
- Assessment and personalized care planning for every child.

Continuing Care Team (CCT)

The Continuing Care Team is responsible for the development, management, and delivery of care to children eligible for Children’s Continuing Care through the local Integrated Care Board. Care is provided year-round by specially trained lifetime support workers and nurses, including overnight care and support within school settings.

Lifetime Psychology Service

Our clinical psychology team works closely with the nursing team to ensure the emotional and psychological well-being of the family. The psychologists offer tailored support for children, their siblings, and other family members involved in their care. Their work includes:
- Helping families cope with diagnoses and future planning.
- Supporting relationships and communication within and outside the family.
- Assisting with managing complex treatments.
- Addressing psychological challenges such as anxiety, trauma, and depression.
- Facilitating group support sessions such as sibling groups, family days, and participation groups.
- Collaborating with professional networks such as schools and hospitals to improve overall quality of life for the child and family.
- Offering support through loss and bereavement.

What We Are Looking For

We are seeking compassionate, skilled, and dedicated Community Paediatric Nurse's to join our team. You will work closely with families to provide clinical care, train caregivers, and offer emotional and psychological support as part of a comprehensive, integrated care approach. This role is ideal for nurses who are passionate about providing holistic care to children with complex health needs within their homes and communities.

Key Responsibilities:
- Deliver clinical tasks safely within the home environment.
- Develop and implement individualized care plans.

Requirements:
- Registered Nurse with valid NMC registration.
- Experience in pediatric nursing, preferably in community or palliative care.
- Excellent communication and interpersonal skills.
- Ability to work autonomously as well as part of a team.
- Commitment to providing high-quality, compassionate care to children and their families.
