We are looking for a forward thinking, conscientious and compassionate Nurse to join our nursing team. You will be registered with NMC and may be newly qualified or already have experience of working within a GP Practice or PCN setting. There is the opportunity to develop within the role and learn new skills.

Main duties of the job

Scope & main responsibilities:

  1. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice. To develop, implement and embed health promotion and wellbeing programmes.
  2. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
  3. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.
  4. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF.
  5. Work within own competence and professional code of conduct as regulated by the NMC.
  6. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  7. Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
  8. To be able to perform all practice nurse responsibilities such as childhood immunisations and cytology.

18-24 hours per week

Interviews will take place week commencing 24th February 2025.

About us

We are a two site practice based in the heart of Burgess Hill, West Sussex. We provide GMS to a registered list of 13,600 patients. We have a team of 5 Partners, 6 associate GPs, a paramedic practitioner, clinical pharmacist, care coordinators, social prescribers, an experienced nurse team including 3 nurses and 3 HCAs and a very experienced and compassionate reception team. We have an experienced senior management team, led by our Practice Manager and we use EMIS clinical system, AccuRx and eConsults.

We are a member practice of the Burgess Hill Villages Primary Care Network, along with three other local practices. We strive to provide the highest level of clinically safe and sustainable service to our patients whilst enabling our practice team to flourish within a supportive and family-friendly environment. We utilise technology to improve our processes and embrace change where it can demonstrate better outcomes for our patients and team.

Job responsibilities

The following are the core and aspirational responsibilities of the Practice Nurse Trainee. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  1. To assess the needs of patients using a variety of means, ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.
  2. To be involved in the implementation and evaluation of individual treatment plans for chronic disease patients that promote health and wellbeing.
  3. Learn effective triage skills, utilising all members of the clinical team.
  4. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF.
  5. Learn how to diagnose and manage acute and chronic conditions.
  6. Learn how to review patient medications to enhance compliance and effectively manage repeat prescribing.
  7. Be able to provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages.
  8. To understand the provision of travel medicine services.
  9. To learn how to request pathology services and process pathology results as required.
  10. To be able to understand the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
  11. To maintain accurate clinical records in conjunction with extant legislation.
  12. To utilise SNOMED CT codes effectively by all members of the team.
  13. To ensure the appropriate maintenance and use of chronic disease register.
  14. To ensure compliance with the practice chaperone policy.
  15. To prioritise health issues and intervene appropriately.
  16. To support the team in dealing with clinical emergencies.
  17. To recognise, assess and refer patients presenting with mental health needs.
  18. To learn how to implement vaccination programmes for adults and children.
  19. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice).
  20. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
  21. To learn how to delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual).
  22. To support the clinical team with all safeguarding matters, in accordance with local and national policies.
  23. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
  24. To deliver opportunistic health promotion where appropriate.
Person SpecificationQualifications
  • Qualified nurse registered with the NMC.
Experience
  • Effective time management.
  • Ability to follow clinical policy & procedure.
  • Strong IT skills (preferably including NHS mail, EMIS, and Teams).
  • Commitment to ongoing CPD.
  • DBS Check.
  • Knowledge of Infection prevention and control.
  • Clinical knowledge in the following:
    • - Wound care/removal of sutures and staples.
    • - ECGs.
    • - Venepuncture.
  • Understanding Chronic disease management awareness.
    • - Diabetes.
    • - CHD.
    • - Asthma.
  • Understand the requirement for PGDs and associated policy.
  • Broad knowledge of clinical governance.
  • Knowledge of public health issues in the local area.
