This expanded service will be working across the integrated pathway from acute to intermediate care services in the community, supporting patients with the care and rehabilitation they need, improving clinical outcomes and patient experience. To provide specialist physiotherapy to patients working cross site, rotating between HIT, RADS and CRT as part of an inter-professional team. This role will require weekend and Bank Holiday working on a rota basis.
Participate in health promotion and education of staff, clients and families.
The post holder will undertake regular continual professional development activities.
Degree or diploma in Physiotherapy
Post-graduate qualification in a relevant field or proven equivalent experience / training
Experience of training/supervision.
Experience within an acute discharge planning/admission avoidance team.
Advanced clinical assessment skills with the ability to verbalise the clinical reasoning process in the management of patients within admission avoidance/discharge planning.
Specialist admission avoidance/discharge planning knowledge and interest.
