Clinical Nurse Specialist

An exciting opportunity has arisen for a motivated and dynamic nurse to work within our Oxygen and COPD Specialist Nursing Teams as a hospital and community clinical nurse specialist. The nurse will be based at the Northern General Hospital.
The candidate will be working with the COPD nursing team undertaking clinical reviews of acute COPD patients following hospital admission, via the respiratory and general medical consultant teams. The candidate will deliver individual patient education, to meet the COPD Quality Standards required by the BTS Audit. This will include smoking cessation, medication review and inhaler technique, pulmonary rehabilitation suitability, self-management advice. The candidate will be involved in the triage of patients from admission lists and will assess COPD patients for community support services and pathways.
To assess for associate services (COPD support discharge and COPD nurse led clinics), maintaining good communication and effective working partnerships with other clinicians, in both the community and secondary care.
The candidate will develop their skills and knowledge in recognising respiratory failure, management, and appropriate treatment of this, whilst supporting palliative care needs.
There will also be the opportunity to work alongside the oxygen nursing team to develop knowledge/ experience in the management of oxygen therapy this may include telephone reviews and clinics. They will become familiar with oxygen equipment, safety concerns and be able to offer advice to families, carers, and other health professionals.
The service operates 7 days a week, 08:30 hours - 16:30 hours however this may require a flexible approach to your working hours on occasions. GRaDE Care Group is comprised of 3 clinical directorates:
- Diabetes and Endocrinology
- Gastroenterology and Hepatology
- Respiratory Medicine
All three directorates operate on the Northern and Central campus' although the acute admitting base for all of the specialties is at the Northern General Hospital. The Care Group has 11 acute base wards at the Northern General Hospital and one acute and one elective base ward at the Central Campus. Outpatient services operate on both campus' and are supported by the Patient Booking Hub at the Royal Hallamshire Hospital. - First level registration
- To have a degree or working towards degree level
- To have completed the national PEAP programme for practice supervisors or equivalent teaching and assessing qualification for those with ENB998.
- COPD or Asthma Diploma
- Non-Medical Prescribing
- Palliative care experience
- Certificate in Spirometry or equivalent
- Enthusiastic and committed to own professional development.
- Can demonstrate good team working.
- Motivated and able to motivate others
- Excellent interpersonal and communication skills, able to communicate effectively with the patient, their families and a wide range of health professionals
- Ability to manage own caseload /workload prioritise effectively.
- Demonstrated ability to learn quickly and keen to take responsibility
- Demonstrated ability to make independent decisions
- Ability to work on own initiative and autonomously
- Ability to support learning and education for patients, carers and families and other allied professionals.
- IT competent.
- Ability to adapt to changes in service demand
- Full UK driving licence and has access to a vehicle.
- Audit or research experience or qualification
- Experience of service development and change principles
- Awareness of use of System 1
- Experience of integrated working across teams.
- Knowledge of management of Respiratory conditions
- Previous experience which demonstrates a specialist interest in managing complex Respiratory patients.
- Evidence of COPD national policies, including NICE & GOLD guidance to inform evidence based care and management.
- Knowledge of clinical governance
- Knowledge and understanding of audit
- Presentation skills
- Understanding and experience in managing change and modernisation techniques
- Extensive post registration experience
- oDemonstrate able post registration experience in COPD.
- Experience of working with people with long-term conditions
- Evidence of working effectively and collaboratively across professional and organisational boundaries
- Experience and involvement in undertaking audit and or research
- MDT experience
