We are looking for a community specialist nurse with minimum of 2 years of clinical experience.

  • You will be assessing patients in this job on your own to determine their acuity and their progress in following their treatment plan or recovery goals.
  • Provide clinically sound interventions such as catheterisation, physical assessments, tissue viability assessment and care, phlebotomy, wound care management and EOLC.
  • Exhibit safe clinical decision-making, including evaluation and diagnostic abilities
  • As a centre of excellence, you will deliver a person-centred approach to their independence and wellbeing.
  • Part of this role will include triaging patients who are referred from different stakeholders such as GPs, London ambulance Services and acute hospitals.
  • Being part of ECP you will work closely with TVN, community therapy, End of Life care amongst others allowing you to make meaningful treatment plans for our patients.

Main duties of the job


  • Undertake physical assessments of patients
  • Provide clinically sound physical health care interventions
  • Provide support to wider MDT
  • Act up as nurse in charge when required
  • Provide supervision and line management to agreed cohort

Qualifications and Training

Essential


  • Registered General Nurse, Adult Branch and current NMC Registration
  • Experience and evidence of teaching and assessing e.g. Preceptorship/Mentor or working towards
  • Commitment to complete Independent Nurse Prescriber course
  • Evidence of recent CPD
  • Ability to work in community setting/home visits

Desirable


  • Advanced clinical skills - ability to assess, diagnose and treat as an independent practitioner minimum level 6 study or similar
  • Independent Nurse Prescriber
  • Advanced clinical assessment skills at minimum level 6 study or similar course or commitment to complete

Experience

Essential


  • Demonstrable experience and work qualification, preferably within an Intermediate Care or community environment.
  • Experience of supervising junior staff in the team
  • Experience of quality improvement processes including audit, risk management and a good understanding of clinical governance and change management
  • Experience of and ability to communicate effectively with variety of partners and stakeholders - verbal, written and electronically
  • Ability to problem solves and prioritise
  • Experience of complaints handling

Desirable


  • Evidence of a range of clinical competencies appropriate to the nurse role e.g. venepuncture, palliative care, IV therapy
