If you are passionate about providing the best possible care and would like the opportunity to empower patients and prepare them for going home, then a career in the Community Care Beds (CCBs) could be the one for you. Home first is the ideal outcome for many patients leaving hospital and providing more care in persons own home is preferred, but a temporary stay in a setting that has a homely environment and that can provide 24-hour care is sometimes required.
The Community care beds do this, enabling early discharge from hospital and reducing hospital admissions. The CCB service delivers complex rehabilitative care for the frailer older adults in Leeds and the nursing care is delivered alongside a wider multi-disciplinary team. The focus always is on the individual, helping them progress to going home.
Community care beds service works on a shift pattern and covers 24 hours, 7 days a week
Community nursing in the care beds suits people who can work independently and within the wider multi-disciplinary team. Youll be resilient, motivated, and adaptable to working in an ever-changing environment and of course, a passion for providing the best possible care to all. You will have great communication skills and maintain a high standard of clinical skills when carrying out nursing procedures. Working for Leeds community Healthcare NHS Trust

The post holder will work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge.
Demonstrates a detailed theoretical and practical knowledge of common aetiologies, pathologies, conditions and presentations involving a critical understanding of theories and principles.
Demonstrates innovative and specialist skills to select methods and tools to holistically assess, plan, implement and evaluate in a complex and specialised field of work.
Makes consistent judgements requiring complex fact analysis, interpretation and comparison of options and ensures clinical reasoning is reflected accurately in case records
Engages and actively involves the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of involvement and empowerment
Works to standards of proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a competent standard
Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all areas of practice including the management of any complaints and compliments.
To comply with the organisations Infection Prevention and Control requirements, including bare below the elbows dress code for staff with a direct care delivery role
Professional degree or diploma in relevant field
Current NMC registration

Working in community, primary care-based teams, Care home team
Experience in leading and managing in teams, delegating, coordinating complex care
