Do you have innovative ideas to help us improve the discharge services for the people within Somerset? An exciting opportunity has arisen to join the Integrated Discharge Team as a Band 6 Discharge Liaison Nurse.


The team are visible through out the Trust and present on every adult inpatient ward and re there to assist when patient no longer meet the Criteria to Reside in the Acute Trust.


We would highly recommend making contact with the team and a shadow shift with the team is possible.
The post will be assisting with discharges in adult inpatient areas, you will be involved in complex discharges, supporting the ward MDT with decision making, supporting the Band 3's within the team.


To provide and maintain a high standard of evidence based, patient centred care.


To demonstrate good interpersonal skills and to develop practical skills required within the specialty, whist reflecting the philosophy of care.


Training will be provided for all the appropriate areas and processes.
