Cotehele is a Functional Mental Health Unit for predominantly over 65s, located in a purpose built 10 bedded Unit at the Glenbourne Unit, Derriford. Cotehele, a mixed ward was specifically designed with separate male and female corridors and individual en-suite rooms. A knowledge of physical and mental health challenges is required for the role. Exciting new changes are ahead as the Home Treatment Team are working with the ward to prevent admissions, support leave and facilitate a quicker discharge. If you would like to be considered as part of the team at this exciting time, then this job might well be for you.

All candidates must be computer literate and have a firm understanding of information governance. If you think you have the right skills to offer us at this exciting time then we would very much like to hear from you. You will need to be able to work a rotating shift pattern to cover our 24 hour 7 day a week..

This post requires the post holder to be able to undertake all statutory and mandatory training including physical intervention training.

In return you will receive enhanced payments for night and weekend work, access to a pension scheme and CPD days each year We will also support and facilitate your development in this role through supervision and a personal development plan tailored to your needs and experience.

Main duties of the job

He / she will be designated to take charge.

The staff nurse will at all times work within the spirit of the NMC code of Professional Conduct for trained nurses and local policies.

All Livewell Southwest staff are expected to able and willing to work across a 7 day service.

Job responsibilities

Key Tasks and Responsibilities of the Post

Utilise nursing skills to assess, plan and implement and deliver care to patients who are in an acute phase of their illness

Organise the provision of care and act as Named Nurse/Associate Nurse for a specified group of patients.

To function as a member of a multidisciplinary team taking appropriate responsibility for the implementation of decisions made by the team

Ensure planned nursing care is implemented and evaluated, take appropriate action in the light of evaluation.

Participate in training programmes for both trained and untrained staff

In conjunction with the university and Senior Staff Nurses ensure training programmes/objectives for learner nurses are implemented.

To be responsible for implementing drug administration procedures according to laid down policy.

Maintain standards of care to highest level within available resources.

Maintain confidentiality in respect to the patient at all times

To actively participate in line management with senior and junior members of staff.

To liaise effectively with other wards and services. To develop and maintain relationships with providers statutory and non-statutory

To maintain confidentiality in respect of the patient/client at all times.

Communicate clearly with patients, their families and all other mental health professional groups about their health problems and care plan.

Actively participate in clinical governance and quality improvement initiatives, e.g. clinical audit or clinical policy development.

Play an active role in CPA multi disciplinary reviews and ward rounds, reporting on the nursing assessment of the patients progress and participating further in M/D Care Planning.

Ensure that patients are encouraged to play a part in the Care Plan and to sign it and ensuring they receive a copy of it. This is to promote person centred planning

Provide mentorship for students

Attend and participate in meetings ie. Case Conference, Mental Health review tribunals

To actively participate and provide clinical supervision

To have a working knowledge of Plymouth Community HealthCare Policies and Guidance.

To actively participate in rotation programme across the Locality

To have a working knowledge of specific policies relating to your work area, i.e. seclusion policy, ECT policy, Safeguarding and be able to apply this in practice.

To work within NMC Code of Conduct in regards to documentation and to ensure compliance with the Trust clinical record keeping policy

To participate and undertake in the appraisal process

To have a competent and robust knowledge of legislation including Mental Health Act, Mental Capacity Act and Deprivation of Liberty

To support the induction and Preceptorship programme

To ensure mandatory training is up to date and be able to undertake in their own work place

To be able to assess and manage risk for their personal safety and the safety of their colleagues within the relevant policies

To work in accordance with the NMC guidelines around CPD and be able to evidence this via a portfolio

To at all times work within the NMC Code of Conduct

To assume responsibility as delegated by a senior member of staff

To participate in coordinator / duty rota as required

To participate in new service developments such as Place of Safety and Nurse Lead discharge

The post holder is required to comply with all infection control, policies and guidance, attend relevant updates and report issues of concern to their immediate line manager (if no action or explanation received, then it is the individuals responsibility to escalate to Director of Operations / Chief Executive office).
