Operating within the framework of relevant legislation, your role will involve utilizing social work experience in a multi-disciplinary team. Main Accountabilities: Manage a caseload and provide advice, therapeutic interventions, and advocacy according to professional expertise for clients and carers. Work with service users who are difficult to engage and have chaotic lifestyles, adapting to unpredictable and challenging situations. Engage in the CPA process to provide enhanced care, including evidence-based psychosocial interventions, coordination of services, family/carers involvement, and needs-based reviews. Negotiate service delivery with providers, ensuring compliance with service level agreements and statutory responsibilities. Monitor progress, amend objectives, and ensure cost-effectiveness and compliance with agreed CPA and Care management Policies and Procedures. Participate in activities benefiting from professional skills, such as working parties, policy formulation, group work, and community development work. Inform the Services Manager of specific needs and challenges in service provision. Undertake investigations and prepare reports for panels, case conferences, and courts. Contribute to standards of practice, training, and development activities, including the supervision of students and care co-coordinators. Collaborate with the CRHTT to facilitate admission and discharge to and from inpatient units. Engage in training activities to support good practice and the development of the whole service.
