The post holder will be part of the Chief Medical Officer's (CMO) structure within the two Local Care Organisations providing knowledge, experience and expertise to the design, implementation and operation of a population health management approach to improving health and wellbeing outcomes for residents in Manchester and Trafford.


The Population Health Management (PHM) Strategic Lead is a significant strategic leadership role at citywide and borough levels to ensure the neighbourhood models focus on population health management (PHM), are co-produced, resident-centric (service design through to evaluation and governance), improve access and address inequities.The role will support the locality level design, development and mobilisation of local and regional strategies and policy changes as they relate to population health management methods. The role will provide management, quality improvement support and leadership to the locality. The postholder will support the design and implementation of policies and pathways for neighbourhood and locality services, focusing on locality issues and providing support to colleagues to find and implement solutions reflecting the needs of the population and promoting best practice.


The PHM Strategic Lead will work in partnership with the Clinical and non clinical Locality Leads and the Locality Assistant Directors as well as other LCO executives, colleagues from public health and locality and regional NHS GM colleagues, developing a close working relationship with Business Intelligence teams to ensure and enable a data led approach.


The postholder will build relationships and work effectively with wider partners and stakeholders in the locality, including in acute care, social care, VCSE sector and community health services to develop and support the delivery of a vision and long term population health improvement strategy for the neighbourhoods.


The PHM Strategic Lead is expected to work across the portfolios of the Chief Medical Officer, the Chief Operating Officer and Director of Strategy of the LCOs, in order to deliver the agenda across integrated primary care and community services.

Provide strategic leadership and management for population health improvement across the two LCOs.


Working with the INT and GP neighbourhood leads, support the neighbourhoods to develop and deliver neighbourhood PHM plans by working with LCO colleagues and wider stakeholders including local residents, councillors and community groups as well as health and social care partners and ensuring the plan is representative of the neighbourhood and aligned to LCOs strategic goals and Outcomes Framework.

Qualifications: Academic / Professional Experience
Significant senior level experience in population health management

Knowledge and Understanding of the Business planning process and its application in service delivery
Extensive knowledge and experience of designing and implementing population health management approaches and improvement methodologies in community services and Primary Care Networks.

Skilled in preparing, and receiving highly complex sensitive and contentious information, negotiating with senior stakeholders on difficult and controversial issues, and presenting complex and sensitive information to large and influential groups.
Strong interpersonal skills and personal credibility, with the ability to quickly gain the confidence of and build relationships with others, including senior stakeholders, managers, clinicians and staff.
Able to develop effective networks, and work in partnership with others to deliver and improve services.
Highly developed strategic leadership skills with the ability to motivate and build effective teams / inspire others including the ability to involve staff and partnership organisations to meet challenging targets and to work with senior and executive leadership.
Involves others in problem analysis and the identification of solutions, where possible enabling individuals or teams to find their own solutions.
Promotes the building of effective teams and collaboration between teams.
Facilitates transformation and change to improve services and outcomes.
Skills and Abilities - Information Management and IT
Well-developed IT skills to manage/ manipulate performance management information and develop meaningful performance reports from this.
Understanding of IT systems and appreciation of their application and management in order to ensure effective PHM strategic and operational delivery.
Critically analyse and interpret complex management and patient information and data sets and make recommendations to others.
Using analytical and decision making skills, demonstrates ability to explore, evaluate and interpret complex information and options and make decisions and recommendations based on an effective analysis of options.
Skills and Abilities - Management
Uses a range of management styles as appropriate.
Experience at senior managerial level within health/ social care delivery including responsibility for strategic planning and large programme delivery.
Preparing and presenting reports for consideration at senior meetings.
Demonstrable success in delivering change and improvements in performance with and through clinical and non-clinical teams. Ensures patient safety by assessing and managing risk associated with service developments.
