Hestia's Mental Health Integrated Voluntary and Community Sector (VCS) Team work within the NHS' Community Mental Health Team (CMHT). As a Pool Worker at Hestia, you will play a crucial role in our Voluntary Community Service . Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.
Work within multidisciplinary team (MDT) of clinicians (Mental Health Nurses, Community Psychiatric Nurses, Psychiatrists, Psychologists, Occupational Therapists) and Social Workers.
To be a named Keyperson on Service User's (SU) electronic records (RiO) and a main point of contact for the SU, as well as relevant statutory and community organisations.
Work collaboratively as part of the Core Locality MDT and SU to develop their recovery plan and lead on co-produced formulation of community/social support related objectives and goals.
To attend all relevant clinical and social MDT appointments relevant to wider MDT team and individual SU's support.
Record SU's support and progress information on the BEH Trust's clinical electronic patient records system (RIO).
To hold a caseload as well as provide ad-hoc joint work support (within workload capacity of 30 Service Users per month).
Lead on assessment and management of client care needs through Dialog +, initial data collection, monitoring of SU's progress, feedback, discussion, and reports and in care reviews with the clinical MDT team.
Lead on building relationship with the local community and statutory services and connect clients to those resources and support a ‘warm handover'.
To be aware of fundamental theory and practice and apply to clinical work applicable in this service in terms of Mentalisation Based Treatment, Structured Clinical Management, Risk Assessment and Management, and Relapse prevention work.
To excel in this role, you should possess:
NVQ Level 4 in care or equivalent OR Minimum of two years working experience in a mental health setting.
Lived experience of mental illness and completed Peer Support training or equivalent.
Understanding of the issues and concerns of mental health service users.
Awareness of treatment outcomes, recovery tools, care planning, assessment of risk.
Experience of training, teaching, coaching/mentoring others.
Appreciation of the community resources within the geographical location of the post and key partners.
