Main duties of the job

The post-holder will be a member of the District Nursing Team and will participate in the clinical case management of patients who have a wide variety of complex, high-risk and supported/self-care conditions. Working directly with the Team Leader, the post-holder will help ensure the provision of a high standard of nursing care and health promotion. The care will be provided in a range of settings including patients' homes, residential homes, nurse led clinics and GP practices.

This will include:

- Holistic assessment, planning, implementation and evaluation of a plan of care for patients with long term conditions, tissue viability, continence, palliative care, and promoting/facilitating patient independence.

- Assists in the delivery of National Service Frameworks.

- Providing public health and health promotion programmes to improve health and reduce inequalities.

- Maintaining contemporaneous written and computerised records.

Job responsibilities

You will be responsible for contributing to joint decision making with colleagues to manage your patients and the good functioning of the team. As there is no hierarchy within the team, you will self-organise to ensure all local management duties are undertaken effectively.

CLINICAL

- To undertake the first contact assessment and review, including planning, implementation, evaluation, using a solution driven and goal setting process

- To actively manage patients with long term conditions and work towards preventing complications to reduce the need for hospital admissions

- To participate in multidisciplinary working involving other agencies as appropriate

- To maintain good working relationships with other health care professionals, statutory and voluntary organisations

- To utilise links with other agencies and service to facilitate appropriate care packages and provide information for clients that focus on promoting independence, self-care and wellness and keep people out of hospital.

- To work in collaboration with own team and others to ensure all patients that require care are supported appropriately.

- To undertake planned and opportunistic health promotion and education activities with patients, carers, family and neighbours

- Maintains close communication with GPs and other members of the Primary health care team to promote effective coordination of care

- To take responsibility for evaluating and reflecting on his/her own practice

- To perform and maintain competence in a wide variety of clinical skills (which includes but is not limited to) administering injections, catheter management, venepuncture, etc.

- To actively participate in Nurse-Led clinics within locality e.g. Continence Clinic, Leg Ulcer Clinic, etc.

- To work collaboratively with allied agencies in meeting the needs of clients with long term illnesses

- To participate in intervision (peer to peer clinical supervision)

EDUCATION AND DEVELOPMENT

- To participate in the learning programmes for students on community placements

- To facilitate the learning of other members of the team

- To champion and actively participate in organisational learning and organisational change

- To take responsibility for your own professional development

- To participate in clinical supervision in accordance with community services within GSTT

- Act as mentor and/or co-mentor to students and junior staff

- To undertake performance appraisal of team members.

SERVICE DEVELOPMENT

- To work across the organisation to support and promote Neighbourhood Nursing, including support to other teams on the journey to self-management

- To support continuous improvement and research through clinical and management activity.

PROFESSIONAL

To adhere to the Nursing and Midwifery Council [NMC] Code of Professional Conduct and be conversant with other relevant advisory papers

TEAM RESPONSIBILITIES

- Responsible for providing day-to-day co-ordination of workload in your own team and network

- Review and agree staffing levels, inc. Bank and Agency requirements

- Participate in recruitment

- Informal performance management and management of behaviours and conduct as a team

- Undertake as a team the process of continuous performance review

- Give and receive feedback in relation to practice, skills, competence and team-working

- Respond to complaints and incidences as required and undertake any relevant investigations

- To actively implement learning from complaints / incidents in your team and the wider network

- To ensure Health and Safety and Risk requirements are met

- To assist in monitoring the quality of the Neighbourhood Nursing Service

- Contribute to the development of quality measures

- To maintain statistical and written records as required by the GSTT

- Attend and participate in locality meetings with other teams as well as wider meetings across Health and Social Care

- To be aware of and work with GSTTs Policies and Procedures

Rotational duties:

The below duties will rotate every three months or as required by the teams. Band 5s will undertake these subject to appropriate support from more experienced members of the team

Mentor: Guide the new starters to the team. Correspond with new starters and back office for preparations to work within the team i.e. phone email. Ensure local inductions are organised for new staff, training sessions arranged and explore competencies and skills assessment performed.

Reporter: To monitor productivity of the team. Monitor annual leave patient facing time, extra hours over-time sickness. To feedback the information gathered every month. How many discharges/referrals every month. Minute taker. Datix investigator, mandatory training, quality dashboard, liaise with back office.

Developer: To monitor and promote team learning and development. Identify team skill needs and look for information/advice and training opportunities, ensure all mandatory training is complete and aware of relevant study days and training to send staff on.

PR: To promote the team. Design and deliver team ethos, delivering training outside team, meeting chair, attends steering group meetings. Represent the team in trust meetings.

Planner: Plan staff and patient needs. Produce four monthly rota allocate scheduled visits, main liaison with E Roster/Back Office. Meeting timer, outcome team meetings on carenotes.

Housekeeper: Keep office tidy and monitor stock. Monitor stock. In charge of kitty for office essentials and buying office essentials. Cleaning when necessary. Keep office neutral all team.

Confidentiality / Data Protection / Freedom of InformationPost holders must maintain the confidentiality of information about patients, staff and other health service business in accordance with the Data Protection Act of 1998. Risk ManagementAll post holders have a responsibility to report risks such as clinical and non-clinical accidents or incidents promptly. They are expected to be familiar with the Trusts use of risk assessments to predict and control risk, as well as the incident reporting system for learning from mistakes and near misses in order to improve services. Post holders must also attend training identified by their manager, or stated by the Trust to be mandatory.
