The service you will workin….

This service is a joint venture with community NHS team,supporting people with dementia who are living in their own homes. Weaim to provide person-centered care planning and support to peoplewith dementia. The service operates across the entire borough ofHounslow and provides a seamless service across allelements.

We provide practical, person-centered support thatpromotes wellbeing, independence, dignity and respect. Working withpeople to identify their strengths, personal goals and aspirations anddevelop holistic, outcomes-focused plans to support people as theymove towards those goals, and prevent people with dementia beingadmitted to hospital.

We empower people to speak up forthemselves, understand their rights and have greater control overtheir lives and provide carers with the opportunity to take time outfrom their caring responsibilities to look after their own wellbeing.We will also work alongside befrienders, volunteers, and peersupporters to deliver activities that promote independence and reducereliance on services.

A day in this role lookslike….

As a Dementia Coordinator at Hestia, you will play acrucial role in our Mental Health and Complex Needs Service. Yourcontributions will directly influence the lives of our service users,guiding them towards a path of empowerment andindependence.

A day will never look the same and some of thethings you can expect to do/be responsible forare:

Supporting people with dementia and their carers intheir homes.

Social work student supervision- training willbe given.

Develop high quality co-produced care plans withthe person with dementia.

Provide holistic integrated supportwork to people with dementia.

Undertake assessments of riskwith due regard to safeguarding.

To ensure that thecommunication needs of service users are met.

To have a goodknowledge of entitlements and eligibility for those living withdementia and their carers, to services such as benefits andrespite.

To identify the needs of carers and signposteffectively.

Effective working as part of amulti-disciplinary team to meet the needs of clients and reducehospital admissions for those living withdementia.

Continuously update knowledge on dementia tomaintain status as Subject Matter Expert.

Good knowledge of how dementia affects peopleand their carers.

At least 2 years of experience of workingwith service users in a social care or health care sector (preferablyolder people).

In depth understanding of support available toolder adults with illnesses such as dementia, such as benefits andsocial support to maintain independence.

The ability to workin a way that promotes and maintains dignity.

Experience ofproviding services to people with dementia in either a health orsocial care environment.

Ability to co-produce care planswith the person with dementia, their families and immediatecarers.

Excellent communication skills.

Knowledgeand understanding of safeguarding issues and ability to address themappropriately.
