Our core services, which we provide to more than 615,000 people, cover more than 2,000 square miles across Devon, while some of our specialist services cover the whole of the peninsula, extending our reach as far as Cornwall and the Isles of Scilly.
We deliver a wide range of emergency, specialist and general medical services through North Devon District Hospital and the Royal Devon and Exeter Hospital (Wonford).
Alongside our two acute hospitals, we provide integrated health and social care services across a variety of settings, including community inpatient hospitals, outpatient clinics, and within peoples own homes. We also offer primary care services, a range of specialist community services, and Sexual Assault Referral Centres (SARC).
Our state-of-the-art equipment, leading technologies and strong links to local universities put us at the forefront of research and innovations, enabling us to provide world-class care to our local communities.
Its never been a more exciting time to join the Royal Devon, as youll help to shape our services as we continue along our integration journey.
Provide expert clinical assessment, treatment and decision-making for patients who are experiencing a health crisis at home and are at risk of hospital admission liaising with Advanced Clinical Specialists for advice and direction if required.
Support all patients on the Urgent Community Services caseload in at least a daily review and have plans in place, either by providing the care planning or supporting other clinicians in their decision-making.
Linking with other Clinicians across the service and within Health and Social Care and primary care.
Provide clinical support to all aspects of the service and processes within urgent care.
patients are able to remain safely at home to avoid a hospital admission.
Promote collaborative working across all areas of short term services, including social care, community services and voluntary sector.
Review requests for urgent placements and night sits to avoid hospital admission and support Community teams and liaise with budget managers to approve.
Provide line management for junior clinical staff within the service. Deliver clinical training to
Support project management teams when implementing new systems and strategies.

5 hours per week, 09:00 - 17:00, Monday to Friday, to include some weekends
