We are looking for an experienced and motivated full time Specialty Doctor to join our Older Adults Community Mental Health Team, based at South Kensington & Chelsea Mental Health Centre.

The Kensington and Chelsea OA CMHT is a well-established multidisciplinary team, and are experienced in integrated multi-agency working. The CMHT acts as a single point of access to the service with referrals being triaged daily.

The post is an established one within the CMHT an has become available due to the current post holder leaving.

Main duties of the job

The post holder will be expected to provide medical input including undertaking mental health act assessments for patients under care of South CMHT under the supervision of the Consultant Psychiatrist. This post is part of a fully integrated multi- disciplinary team as well as support and administrative services.

About us

Kensington and Chelsea (K& C) adult mental health service provides a range of mental health services for adults within the community and in hospital settings.

These are integrated health and social care services and are based at:o St Charles Hospital - 8 inpatient wards two adult PICUs (male and female), 4 adult acute wards and two Older Adult wardso South Kensington and Chelsea Mental Health Centre, next to C& W Hospital - liaison service, health psychology, therapies team, memory service, older and younger adult psychiatric liaison serviceso Community team bases for adult CMHTs and Psychotherapy services.o A Primary Care Mental Health team (Community Living Well)

The K& C Community Mental Health service is currently going through community transformation process in line with the NHS Long Term Plan. This is a new model to deliver care with greater community integration, focus on rehabilitating patients to transition down the care pathway and delivering acute community care.

The community team provides assessments and continuing care support to service users with complex psychiatric needs in South Kensington and Chelsea operating a 'zoning' model with the aim of stabilising service users sufficiently to enable transfer back to the care of their GP /primary care liaison team although also providing ongoing intensive support to those service users with very complex needs. The team is multidisciplinary with all patients having an allocated worker

Job description

Job responsibilities

1. To conduct initial assessments and follow-up reviews of patients referred with mental illness that require secondary care.2. Establishing, initiating and evaluating treatment plans and risk assessments in collaboration with the multi-disciplinary team and under the supervision of the Consultant Psychiatrist.3. The formulation of specialist opinion and writing of psychiatric reports

4. Undertaking the relevant referrals to other disciplines and services both within the multi-disciplinary team as well as to services and resources external to the service.5. Observation of Trust, K& C MHS and local policies and procedures including those related to medicines management and physical healthcare.6. Interviewing relatives and informal carers of patients as required.7. The post holder will be expected to provide support, teaching and supervision for MDT staff and teaching of medical students.

Qualifications

Essential

  • MBBS (or equivalent overseas)
  • Full GMC Registration
  • GMC Licence to Practise

Desirable

  • Attended an accredited MRCPsych training course (or equivalent).
  • Section 12 approved
  • MRCPsych Part 1 or Part 2 (or equivalent). Absence would not preclude an appointment.

Experience

Essential

  • Completed at least 3 years full time post graduate training (or its equivalent on a part time or flexible basis) at least two of which will be in a specialty training programme in a relevant specialty or a fixed term trainee or have equivalent experience and competences.

Desirable

  • Previous Experience in community psychiatry

Skills and Knowledge

  • Able to communicate clearly both in written and spoken English
  • Demonstrates sound knowledge and skill base with the ability to prioritise clinical need.
  • Demonstrates ability in basic skills i.e. history taking, examination (mental & physical) and the utilisation of external sources of information in order to develop care and treatment plans to meet patient needs.
  • Capacity to utilise a range of approaches to solving problems and making decisions
  • Demonstrates understanding of the principles of audit and research within psychiatry and their application to clinical practice
  • Able to work cooperatively as part of a team and build and maintain successful working relationships with staff in a variety of different roles.
  • Ability to recognise and manage own stress utilising the resources available.
  • Able to work in an efficient, systematic and flexible manner
  • Able to deal with difficult situations and to address them in a responsible and professional manner
  • Understanding of the complex nature of patient confidentiality
