This is a fantastic opportunity for a Registered General Nurse looking to further develop their knowledge and experience in wound management and leg ulcer management with skills including ABPIs. This is to support the existing Team Leaders and the Tissue Viability Lead.

The Wound Care & Lymphoedema Service delivers care to patients registered with a Bury GP who require wound management following surgery, trauma or for skin care advice. The Service provides evidence based comprehensive assessment, management, treatment and prevention strategies for patients with wounds, leg ulcers and lymphoedema. The Service promotes a self-management pathway with patients and the post holder will be involved in facilitating staff to empower patients and carers to self-manage.

The Service is comprised of a team of 14 and includes the Tissue Viability Nurse, Community Nurse Team Leaders, Community Staff Nurses and Health Care Support Workers. The Service operates 7 days per week in clinic settings across the Borough also joint assessments are undertaken with Community Clinicians including Nursing Homes and Adult Community Nursing Teams as required.

Main duties of the job

Assess, plan and implement care maintaining appropriate documentation both electronic and paper

To manage clinic attendance by utilising Northern Care Alliance NHS Group current IT systems

To promote the highest standard of patient care, utilising resources available to the benefit of the patient

To mentor and coach both junior staff and students attending the Service

To carry out complex nursing procedures that would enhance patient care, e.g. ABPIs

To recognise and act on situations that may be detrimental to the health and wellbeing of the patient

To prescribe treatments according to the Trust Wound Care Formulary ensuring budgetary awareness

To actively participate in promoting self management for appropriate patients
