The team ethos is to enable and support people to remain in their own home as the first alternative to inpatient admission.

The Neighbourhood Team consists of Occupational Therapists, Community Nurses, Physiotherapists, Senior Nurse Practitioner, Healthcare Assistants and Administrators. GP and social care colleagues work closely alongside.

The Neighbourhood Team operates from 7:30am to 18:30pm on a seven-day rota including weekends and Bank Holidays. You will be supported to work flexibly within these hours.

Main duties of the job

Your typical working day will include triage and allocation of planned and unplanned visit requests for occupational therapy interventions. This may involve urgent response assessment as part of the teams admission avoidance remit, supporting complex hospital discharges and planning domiciliary interventions to facilitate independent living. You will work alongside occupational therapy, nursing and physiotherapy colleagues in the same team. The role includes mentoring occupational therapy students. We have close links with Worcester and Coventry universities in support of these student placements.

Job description

Job responsibilities

For full details of the duties and criteria for the role please refer to the job description and person specification attached.

The post holder will work as an autonomous practitioner within an interdisciplinary Neighbourhood team providing rapid and expert professional assessment and treatment interventions to patients either in their own homes or other community settings to avoid inappropriate hospital admission or to facilitate early discharge. This will be achieved through a combination of hands on expert clinical skills as an Occupational Therapist with knowledge to treat, educate and support patients and by working with patients and their families to assess, plan and implement appropriate packages of care and treatment

Provide case management for a designated caseload of patients who have immediate and / or complex care needs.

Triage patients to ensure prioritization of clinical need.

Work holistically with all team members, carrying out joint visits and actively participating in the integration of services increasing the knowledge of all team members through case discussion and joint working.

Qualifications

Essential


  • Diploma/ degree in Occupational Therapy
  • Health Professionals Council registration
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer

Desirable


  • Clinical educators qualification
  • Relevant validated clinical courses

Experience

Essential


  • Application of the Occupational Therapy process
  • Use of functional assessment and activity analysis

Desirable


  • Experience of working in an integrated team
  • Experience implementing change in clinical practice
  • Experience in rehabilitation settings

Knowledge

Essential


  • A range of evidence based Occupational Therapy clinical skills and knowledge appropriate to specialism
  • Understanding of professional and ethical boundaries and code of conduct
  • Knowledge of the roles and responsibilities of the professional groups within an integrated team
  • An understanding of the approaches and models of practice relevant to practice.
  • A good level of knowledge in a wide range of physical and mental health conditions
  • Knowledge of rehabilitation concepts and services
  • Good level of knowledge relating to disability, equipment and adaptations

  • A working knowledge of Occupational Therapy Outcome Measures and Models of Practice

  • Ability to communicate clearly and effectively in English in both verbal and written formats
