We are looking for an enthusiastic and dynamic Clinical Nurse Specialist (CNS) or Clinical Paramedic Specialist (CPS) to join our Outreach Team. You will provide specialist palliative care support to patients living in the community, in their own homes, and in care homes/nursing homes.
Position: Band 7 Clinical Nurse or Paramedic
Specialisation: Palliative Care (Experience or Qualifications required)
Employment Type: Full-time and Part-time positions available
Location: Kent (Free on-site parking)
Rates: Competitive locum rates, either PAYE or Limited company
Serving as the initial point of contact for new community referrals and telephone inquiries.
- Participating in a rota of clinicians covering a 7-day service.
- Providing advice and care to patients in clinic and home settings, as well as remotely via telephone and videoconferencing.
- Focusing on individualized care, improving quality of life, and achieving patient-centered goals.
- Working alongside other community teams and specialists to optimize patient experience and enhance team knowledge and development.
Proven community or in-patient palliative care experience at Band 6 or 7, or equivalent experience in areas such as Paramedic, General Medicine, Neurology, Emergency Medicine, Frailty, Respiratory Care, Cardiology, or Oncology.
