The post holder will utilise research evidence based knowledge to act as an information resource for patients; multi-disciplinary colleagues and other stakeholders.

The post holder will hold responsibility for signposting and referring those individuals, families, carers and communities with whom they work to the most appropriate agencies/ services to support their needs.

The post holder will be responsible for designing, delivering and evaluating programmes of education, training and development on a one to one and group basis to MDT team members, and as required organisationally, systemwide and nationally, utilising professional presentation equipment and IT systems as appropriate.

The post holder will be expected to communicate effectively verbally and in writing with colleagues and stakeholders, utilising electronic methods of communication within the remit of the Data Protection Act ensuring that clinical information is conveyed within a timeframe that ensures patient safety.

The post holder will work collaboratively across primary and secondary care to establish and maintain communication to enhance patient experience.

Main duties of the job
  • To work autonomously as an Advanced Clinical Practitioner, using advanced clinical skills to assess, investigate, diagnose, treat and manage patients. Such patients may have co-existing and diverse physical, psychological and social factors, making their management complex and multi-factorial.
  • To develop, utilise and maintain skills beyond the scope of normal Nurse/AHP practice, e.g. advanced physical assessment, diagnostics and prescribing.
  • To plan and direct the management of patients including accessing services within the multi-disciplinary team and wider health, social care, education, vocational and voluntary sector services as appropriate to role.
  • To lead on the strategic development of trust-wide services in their specialist area, working with other strategic and clinical leads as appropriate.
  • To lead and actively participate in education programmes designed to support other clinical staff in skills relating to the speciality.
  • To provide peer support to clinical staff working with patients from the direct caseload or those involved in the management of patients demonstrating conditions relating to the speciality.
  • To assist in the planning, co-ordinating and development of the service, including the evaluation of any newly implemented service plans.
  • To be responsible for insuring that relevant legal and regulatory frameworks are acknowledged through local governance procedures.

Job responsibilities

a) Use advanced skills and expert knowledge to assess the physical and psychosocial needs of a defined client group, developing individualised, patient centred Nursing/ACP goals using a shared decision making approach and provide treatment and therapy, based on the best available evidence, in order to improve health outcomes.b) Provide senior clinical leadership and expertise in the assessment and triage of patients' needs in complex and often deteriorating clinical situations, providing joint assessment within the MDT where appropriate.c) Act as first contact, exercising a high degree of professional autonomy and critical judgement in providing expert advanced clinical assessment andtherapy/rehabilitation for patients with often undiagnosed and undifferentiated needs.d) Initiate appropriate investigations, interpret diagnostic results and determine differential diagnosis, seeking advice and support from medical and other professional colleagues as required.e) Make direct referrals for medical assessment and diagnostic procedures within agreed pathways.

Person Specification Qualifications and/or professional registration
  • Completed MSc in Advanced
  • Clinical practice or equivalent level of knowledge and experience
  • Professional qualification/degree appropriate for ACP role
  • Current Registration with NMC/HCPC
  • Evidence of study at level 7
  • Evidence of recent CPD in relevant field
  • Independent/Supplementary Prescribing (HCPC V300) or Advanced Medicines Management module
  • Teaching qualification (eg PGCert)
  • Coaching or Mentoring qualification
Experience
  • Substantial post registration experience in relevant clinical fields
  • Leadership/Management experience at band 7 or above
  • Experience of teaching and mentoring of staff at all levels across a multi-disciplinary team
  • Experience of involvement in clinical steering groups/taskforces at organisational level
  • Experience of leading, planning and completing audit/research
  • Experience of involvement in clinical or professional groups/networks at ICS or national level
Skills and knowledge
  • Advanced clinical skills in a clinical speciality relevant to role e.g. frailty, respiratory, neurology
  • Delivery of evidenced based care for complex and deteriorating patient presentations
  • Advanced communication skills, including presentation skills, adapted to needs of audience
  • Leadership skills with ability to engage and deliver change through listening, negotiation and partnership working
  • Ability to develop and sustain partnership working with individuals and teams across the organisation and local healthcare system
  • Evidence of service development using quality improvement methodology
  • Ability to manage high pressure situations and prioritise workload to ensure balance between the clinical and other elements of an advanced clinical practitioner role.
  • In-depth knowledge of recent NHS and health and social care legislation relevant to community services
