Role: Social Worker Hospital Discharge Team Pay Rate: £35-£38 per hour About the Role: As a Social Worker in the Hospital Discharge Team, you will play a crucial role in facilitating safe and timely discharges for patients. Your responsibilities will include assessing the needs of patients and their families, coordinating with healthcare professionals, and ensuring that appropriate support services are in place for a smooth transition from hospital to home or other care settings. Key Responsibilities: Conduct assessments to determine patients' needs and develop care plans.
