You will be responsible for supporting and promotion of effective and efficient management of the clinical care environment and the delivery of care, also for ensuring effective communication is maintained at all times, whilst providing support for junior staff.


You will deputise for the ward manager in their absence and will provide clinical leadership and supervision of nursing staff supporting a multi-disciplinary and collaborative approach to nursing care.
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.


We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.


During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

To support the Ward / Team manager in the day-to-day operational functioning of the unit.
Be responsible for the assessment, planning, treatment and evaluation and monitoring of person-centred programmes of care for designated patients across the clinical area, Is responsible for the prioritisation of the care requirements for identified patients
Ensure that the management of verbal and physical aggression, sexually diverse behaviour and vulnerability are sensitively handled in line with policy and procedure, and that the individuals dignity and privacy is maintained at all times.
Is responsible for ensuring continuity of care in accordance with the care programme, seeking support from other specialist practitioners/operational managers as and when required.
Supports the management of clinical / patient risk assessments and develop appropriate actions to support the delivery and maintenance of safe and effective care.
Is responsible for maintaining own accurate health records in patients health care settings in line with Caldicott and Trust Policies and Procedures, and for the monitoring of others within the team. Oversees and supports the placement of pre-registration nurse students and contribute to the learning environment for observers from other health disciplines, supports new staff induction to create a workplace and culture of continuous learning and evidence-based practice. Is responsible for the monitoring of work and performance of colleagues providing constructive feedback, advice and support
Manages staffing rotas within the clinical area, escalating issues when required to maintain the safety of patients / clients.
Contributes to Public Health and Health Promotion initiatives and provides information to support wellbeing and independence for patients/relatives and carers and supports junior staff to do so.
Participate in routine screening for chronic disease management and record results for monitoring purposes to ensure compliance with the National Service Frameworks.
Undertake training relevant to the discipline which has been jointly agreed and identified via the appraisal process
Have an awareness of what is influencing nursing services locally and nationally.
Patient Care
Respond safely and effectively to issues appertaining to the application of the mental health act, including the application of control and restraint techniques.
Work clinically whilst supervising and supporting the clinical development of colleagues to provide nursing care to individuals making use of current evidence to inform the delivery and promote the development of clinically effective and safe care.
Provides professional and specialist advice and leadership on the provision of nursing care within the service.
Is responsible for the monitoring and continual improvement of quality and effectiveness of nursing care delivered in the clinical area
supports patients/service users /carers encouraging them to promote their own health and wellbeing and to express their interests and concerns
To undertake and ensure nursing interventions are consistent with evidence-based practice, transferring and applying knowledge and skills to meet patients needs whilst respecting their need for privacy and dignity
To maintain accurate and legible patient notes (written and electronic) in accordance with Trust and national professional policies and guidelines
Awareness and understanding of the Mental Health Act and its administration and application in practice
Knowledge of local and national priorities.
Dementia experience or desire to work with individuals with Dementia
Understanding of the NMC Code of Conduct and implications for practice

Education, Qualifications and Training
Post registration/ graduate qualification relevant to field of practice
RN Level 1 - Learning Disability or Mental Health with experience at band 5
