Community Children's Nurse

An exciting opportunity has arisen to join our friendly and progressive Community Children's Nursing Team, Full-Time (37.5 hours per week) as Band 6 Nurse. The post sits in a well-established friendly CCN team that delivers acute, specialist school, ambulatory and chronic, complex care providing a 5 day service to children and their families living in Harrow Borough. The post holder will work across the Community Children's and Specialist School Nursing team. You will not be required to work weekends and bank holidays currently. However, this may be required in future as the service is looking to provide a 7 day week service. This challenging and rewarding role offers the opportunity to meet the needs of children, young people and their families within a multidisciplinary inter agency framework.
Main duties of the job We are looking for an enthusiastic forward thinking children's nurse, community experience would be an advantage but not essential. You will be a highly motivated independent practitioner with sound evidence based skills. The post holder will manage a varied caseload, act as a resource for an identified area of clinical special interest and contribute to the on-going development of the CCN and SSN team. The successful applicant may have contact with patients or service users. Car owner, driver essential Job description Job responsibilities You will have the ability to provide case management of an identified caseload of children and young people with complex needs by utilising and promoting evidence-based care. You should also have excellent interpersonal skills to support you in managing complex discussions with a range of agencies and individuals, including care providers, patients and relatives. In addition theability to work in partnership with parents and multi-disciplinary agencies to provide skilled care to children and young people within the community to support safe discharge and hospital admission avoidance, is essential. You will be highly organised and have a flexible approach to work, able to deal with difficult situations autonomously with demonstrable, excellent communication skills. This role also requires the post holder be flexible around working days, as support to staff may be required in the evening or nights, as part of planned supervision and support. - Epilepsy PET 1 & 2 course Independent & Supplementary Prescribing Course
- Registered Children's Nurse RSCN
- Numeracy skills required for the safe administration of medicines
- Experience as band 6/band 5
- Recent experience within an accute/community setting
- Community experience
- Special school experience
- Able to communicate with the multi-disciplinary team, present cases, take note of actions, update others in the team and use handover effectively. Speaking face to face but also by telephone and writing letters and reports.
- Care planning: being able to assess patients, develop, implement and evaluate a care plan
- Medicine management/administration
- Safeguarding and advocacy
- Experience of working with accute and long term conditions
- Experience of teaching, mentoring and supporting others in the team
- Empowering and working in partnership
- Understanding how the Mental Capacity Act and Deprivation of Liberty Safeguards impacts on their work and the people they are working with

- Infection control and Aseptic Non Touch Technique
- Manual Handling patients

- Maintaining accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the CNWL policies
- Work well with other members of the team including communicating effectively, being kind and considerate, reliable and enthusiastic
- Manage others and contribute to the management of the team
- Contribute to the culture of the team and play a role in making it inclusive, kind and hard working
- Coordinate work allocation and shifts (if appropriate)
- Intravenous taining theory and practice
- Ability to work autonomously/prioritise work load
- Expertise in particular skills and evidence of how you developed them and use them in your current role
- An understanding of clinical governance framework in relation to practice
- Ability to work under the direction and guidance of senior team members

- HR skills training e.g. recruitment, managing absence
- Awareness of the health needs of children and young people
- Access to a car or other motor vehicle for use for work purposes
