Qualified Social Worker - Community Mental Health Team
Post Pay Level: 7 - £38,223 to £43,421 plus £1039 Local Weighting Allowance per annum
Location: Slough Community
As a Social Worker within the Slough Community Mental Health Team (CMHT), you will work as a vital member of a specialist mental health team, focusing on assessment, care planning, and service purchasing. Operating within the framework of relevant legislation, your role will involve utilizing social work experience in a multi-disciplinary team.
Engage in the CPA process to provide enhanced care, including evidence-based psychosocial interventions, coordination of services, family/carers involvement, and needs-based reviews.
Monitor progress, amend objectives, and ensure cost-effectiveness and compliance with agreed CPA and Care management Policies and Procedures.
Participate in activities benefiting from professional skills, such as working parties, policy formulation, group work, and community development work.
Contribute to standards of practice, training, and development activities, including the supervision of students and care co-coordinators.
Collaborate with the CRHTT to facilitate admission and discharge to and from inpatient units.
Engage in training activities to support good practice and the development of the whole service.
Qualified Social Worker with a recognized professional qualification (e.g., BA Social Work, MA Social Work, DipSW): Current registration with Social Work England: Experience of working with Mental Health and Child protection: Ability to work with staff and managers at all levels and across Health and Social Service disciplines: Ability to use IT packages, including Word and Excel: Ability to converse at ease with customers and provide advice in accurate spoken English: Understanding of working with individuals and adults who have drug/alcohol problems: Knowledge and understanding of Child and Community Care Legislation, including the Mental Health Act 1983: Understanding of what constitutes a high-quality service: Understanding and commitment to equality and diversity: Recognized social work qualification CQSW CSS, Dip SW: Essential
