We're on the lookout for dynamic Mental Health Practitioners/CPNs to join our vibrant Community Older Persons Mental Health teams in Wells and Frome, Somerset.

This opportunity could suit those at Band 5 level, with the potential to progress to Band 6 within the year - an ideal chance for growth and advancement.

We're seeking registered Mental Health Nurses or Registered General Nurses with Mental Health experience to join our fantastic team across the Mendip Neighbourhood. We would ideally like the successful applicant to complete the non medical prescribing course but it is not essential.

You'll be at the forefront of assessing patients referred to the Older Persons' Mental Health Service, using a recovery-oriented approach and strengths model. You'll assess, plan, implement and evaluate treatment safely and effectively, in partnership with medical team and nurses, service user and carers, using evidence based practice.

We operate Monday to Friday, but flexibility may be required to meet the needs of our the service. A valid driving licence and access to a vehicle is required for the role.

Main duties of the job

You'll undertake the assessment of risk and nursing needs and contribute to the multi-disciplinary team care plan, memory assessments, under the development and implementation of individual care plans for the client.

As part of our Neighbourhood Team, you'll play a crucial role in developing new systems and processes to enhance community services integration. Key responsibilities include acting as a Key Worker for a defined caseload, focusing on individuals with severe and enduring mental health issues andother complex needs and difficulties in the community and hospital settings.

Furthermore, you'll contribute to the Therapeutic Group provision for Mendip OPMH patients and actively participate in Neighbourhood and Multi-Disciplinary Team meetings, discussing shared patients, including care home resident. Your dedication to delivering high-quality patient care, flexibility in working across various community settings, and commitment to supporting patients and their families/carers will be paramount in this role.

Job responsibilities

In this role, you'll take on a vital responsibility within the community, working closely with individuals and their loved ones who face multiple and complex challenges. As an integral part of the multidisciplinary team, you'll conduct assessments and provide psycho-social, evidence-based interventions using a collaborative approach to recovery. Your expertise in positive risk management will ensure the safety and well-being of those under your care.

Your main focus will be on conducting mental health assessments and supporting patients as they work towards their recovery goals. Additionally, you'll play a role in facilitating therapeutic groups aimed at promoting well-being, including sessions on Living well with Dementia, Emotional Skills, and Cognitive Stimulation.

You may be called upon to conduct memory assessments and provide support to individuals diagnosed with memory impairments.

  • oRelevant degree or equivalent experience
  • oExperience as a Band 5 or equivalent Mental Health Professional
  • oMust be a car driver with a valid driving licence and have access to transport with appropriate business insurance in order to travel throughout the trust to meet the needs of the service
