Band 6 Specialist Mental Health Pharmacist
We are working with a local NHS who are seeking a Specialist Mental Health Pharmacist to join their team in Shrewsbury.
Location: A local Mental Health Hospital in Shrewsbury
Rate of pay: £30p/h Umbrella (mileage paid from home to site)
Working Hours: Full/ part time
Contract: 3 month initial contract with the view of extension
Key responsibilities:
* Provide pharmaceutical input at multi-disciplinary team meetings, rapid review meetings and community team meetings where necessary undertaking patient medication histories and devising pharmaceutical care plans.
* Provide specialist advice and information about medicines and their use to patients carers and the public.
* Conduct regular 1:1 consultations about medicines with patients and carers. Promote the concept of shared decision making and patient choice at all times.
* Visit allocated wards and community team bases to regularly review medicines cards and professionally check prescriptions, thereby ensuring compliance with UK and European medicines legislation, SPCs, national guidance, MPFT policies, procedures, formulary and guidelines.
Requirements:
* Pharmacist registered with the GPhC
* Mental Health experience
