You will work in a multi-disciplinary way alongside a range of specialist services, including those in the community, social and voluntary services. The team work closely with GPs and developing these relationships for the benefit of our patients is a key part of the role.We have an excellent learning and development department. Our aim is to provide high quality, safe services that improve the health, wellbeing and independence of the people entrusted to our care. You will feel a real sense of achievement and that your work makes a difference to the lives of local people. You will be responsible for managing your allocated work on a day to day basis ensuring patients are prioritised according to clinical need and that holistic assessment and care plans are recorded in line with the Trusts standards. The role will also develop you to managing the co-ordination of a shift and supporting and line managing junior staff.

Main duties of the job

Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, mental health, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.

The post holder will have responsibility for managing, co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.

The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.

The transformation will happen in staggered stages Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
