You will work with colleagues across health, social care, mental health and the voluntary sector to ensure holistic and joined up delivery of care.


You will empower patients to make decisions about their own care and encourage them to exercise their rights and choices, signposting them to relevant community and voluntary services and supporting patient's carers and members of the integrated team with management of urgent presentations
To utilise clinical expertise, function as an experienced clinician managing the assessment, diagnosis and treatment for patients with a range of urgent care presentations.


To have clinical autonomy regarding the clinical diagnosis and future clinical management of presenting patients through clinical assessment and the requesting and interpretation of the results of specific medical investigations


Support patients with advice, information, persuasion and direction to community resources to encourage self-management and ongoing independence


Provide support, training and advice to patients, parents and carers


To maintain appropriate clinical records in line with current practice guidelines

Professional knowledge and skills underpinned by theory and experience, acquired through degree supplemented by specialist training, experience in specialist field
