Our Associate Clinical Case Managers support some of themost vulnerable children, young people and adults following lifechanging injuries such as acquired brain injuries, spinal cordinjuries, complex orthopaedic injuries and more.

Our AssociateCase Managers operate UK wide, and due to growth; we are seekingclinically qualified professionals to join our network on aself-employed basis:

What youll be doing as AssociateClinical Case Manager:

  • managing a case load ofclients through their rehabilitation.
  • leading on assessmentin the home and their rehabilitation and goal setting.
  • signposting and advocating for what they need, and assessingwhat needs to be in place for the people you support to live afulfilled life, whether that be accessing services, reintroductioninto their communities and social lives, accessing work or meaningfulactivity and so on.
  • Managing all administration in relationto cases, including sending instructions to third parties and writingand reviewing reports.
  • To be successful in this role, youllhave proven clinical experience and a valid clinical registration.Youll have previous experience putting together and implementing
  • rehabilitation packages and strong relationship buildingskills. As this role is field based, you will be a driver with accessto your own vehicle and able to travel across the UK. Were happy tosupport you as a new case manager, but if you have previous similarexperience in the community then wed love to hear from you, too.

  • Work that is Monday to Friday, 9am5pm.

Our associate case managers are some of the bestclinicians and case managers across the UK. We believe this is becauseof their extensive knowledge of catastrophic injury, rehabilitationand wider support services but also because they care about rebuildinglives and creating a fulfilling future for those we support.

As an associate case manager you willwork with people who have suffered catastrophic injury to:

  • Assess their needs, understand them pre-injury and identifytheir needs post-injury
  • Put plans in place based on theseneeds for a successful rehabilitation with clear, manageable goals
  • Implement the plans alongside the client (including theparent or carer of children and young people)
  • Coordinate therelevant care and support whether that be accessing statutoryservices, therapy and rehabilitation, financial support as well aseducational needs, vocational support, hobbies and lifestyle choices
  • Monitor progress and respond to changing needs.

You may also have experience in the following: OccupationalTherapist, Mental Health Occupational Therapist, Home ModificationSpecialist, Assistive Technology Specialist, Sensory IntegrationTherapist, Work Capacity Evaluator, Certified Occupational TherapyAssistant (COTA), Industrial Rehabilitation Specialist, RehabilitationServices Director, Community Integration Specialist.
