Calderdale and Huddersfield NHS Foundation Trust are seeking an enthusiastic Advanced Clinical Practitioner to join our Medical Same Day Emergency Care Unit.


The Advanced Clinical Practitioner will be a highly experienced, knowledgeable and educated member of the team who is able to work autonomously making high level decisions to treat health needs. Decide on and carry out treatment, which may include the prescribing of medicines, or referring patients to an appropriate specialist.
Work autonomously within a wider health care team and as a lonepractitioner.

To work closely with acute medical teams to provide effective and reactive services for people presenting to the Same Day Emergency Care Unit to ensure rapid delivery of treatment, care planning and hospital admission prevention where appropriate.
Work clinically to optimise health and wellbeing, through comprehensive history taking and physical assessment of patients.
Demonstrating a critical understanding of own level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information
To prescribe and review medication (as an independent prescriber) for therapeutic effectiveness appropriate to patient need and in accordance with best/evidence-based practice and national and local protocols and within the roles scope of practice and legal framework
Initiate pharmacological intervention, using non-medical prescribing or patient group directives and administer medications as prescribed by others in accordance with the CHFT policies
Accurately request radiological and pathology investigations and make clinical decisions based upon the results.
To be responsible for the delivery of a detailed, accurate and potentially complex clinical handover and to produce detailed and accurate written information within the medical notes regarding all clinical assessments, investigations completed and requested, and treatments administered.
To advise, communicate and work in conjunction with a wide range of clinical colleagues specifically, primary care and community teams and Social Care professionals, leading and facilitating a patient or client focused, coordinated case management approach across primary and secondary care for people who are most vulnerable to, and at high risk of repeat admissions to hospital.
Take a lead in empowering and facilitating other professions to meet the care needs of service users and their carers by the promotion of principles and philosophies such as care closer to home, rehabilitation and optimal self-care.
To proactively participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through the proactive provision of care in or as close to the patients own home as possible.
Provide specialist advice and support to patients, their families and other healthcare professionals following assessment, diagnosis, treatment, and discharge/admission.
Lead the development, implementation and audit of protocols and guidelines.
Establish and maintain a regional and national network of contacts relevant to the service.
Work in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use.
Communicate effectively to develop and sustain partnership working with individuals, groups, communities and agencies.
