Advanced Clinical Pharmacist - Community Mental Health Services Tees Esk and Wear Valleys NHS Foundation Trust

We are looking for experienced Advanced Clinical Pharmacists to join our community services in our County Durham and Darlington locality. These are exciting times in Tees, Esk and Wear Valleys NHSFT as we expand our Medicines Optimisation services into our community teams across the organisation and into primary care services.

We have 2 roles initially on a fixed term. It is anticipated that successful implementation of the roles will result in on-going funding.

Each role will cover a geographical area of County Durham to support community mental health teams and primary care networks as follows

  • Sedgefield and Durham Dales area
  • Central Durham and East Durham area

We envisage the role will become embedded within community teams delivering medicines management services for their patients and supporting the repeat prescription of medicine under shared care arrangements, working alongside a senior pharmacy technician and other healthcare professionals. Previous mental health experience is not essential as training will be provided.

Main duties of the job

If you are a pharmacist experienced in or interested in mental health this could be the post for you, as we transform our medicines related services for our community patients. This is a very patient focused role, with an emphasis on improving the processes to reduce any risks with medicines and enhance the patient experience especially at interfaces of care; aswell as ongoing monitoring and supply of medication. Whilst based in the community there will be clear links for this post to our inpatient and dispensary teams based Darlington; plus inpatient pharmacy teams based in Durham, aswell as to the wider Pharmacy Leadership Team.

The successful applicant will share our vision and be committed to our values and goals for the delivery of the best care to our patients. You must have current registration with The General Pharmaceutical Council, have a clinical diploma and ideally have substantial clinical pharmacy experience gained in secondary and / or primary care. We are committed to developing our staff and there will be an opportunity to undertake the Post Graduate Certificate in Psychiatric Therapeutics, Non-Medical Prescribing certificate (if you are not already a prescriber); as well as developing your leadership knowledge and skills.

About us

The Trust is currently working with staff and partners to deliver the Community Mental Health Transformation programme (CMHTP). This is a five-year programme, driven by the NHS England Long Term Plan, that looks at the needs of local populations and how organisations can move towards a more joined up approach to services to improve the lives of people living with mental health illnesses.

This is an exciting time to be involved in the transformation of how mental health services are delivered in the community and you will be instrumental in providing medicines management and optimisation services for the benefit of service users and the wider local population, as well as supporting our wider partners in primary care.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

A lease car scheme is in operation.

The ability to work flexibly and manage your own workload is essential. You will be assigned a team base in the vicinity of the community team(s) you are working with.

Job responsibilities

To be responsible for a specific aspect of the clinical pharmacy service and take a lead role in medicines optimisation within their designated area.

To work with pharmacy professionals, clinical staff, patients and carers in the provision of a trust wide clinical pharmacy service in all care settings, ensuring high quality patient focused pharmaceutical care

To participate in the development and delivery of the Medicines Hub, to ensure continuity of supply medicines and support patients as they move between episodes of care and care settings.

To use non-medical prescribing skills (if qualified and within scope of practice) to support safe prescribing and administration of medicines.

To support the Deputy Chief Pharmacists and Lead Pharmacists and be involved in pharmacy service developments as needs or opportunities arise.

To provide both clinical and day to day supervision for Senior Pharmacy Technicians, Rotational & Specialist Clinical Pharmacists in addition to other healthcare professionals when required

To provide day to day leadership, working closely with Lead Pharmacist and Lead Pharmacy Technician in their locality, deputising for the Lead Pharmacists as required

To act as a line manager for Senior Pharmacy Technicians - community and/or Foundation / Specialist Pharmacists as appropriate.

To promote at all times a positive image of people with mental health conditions and learning disabilities.

To promote at all times a positive image of the Pharmacy and the wider Trust.

Person Specification Qualifications / registration
  • Masters degree in Pharmacy or equivalent
  • Member of the General Pharmaceutical Council (GPhC)
  • Postgraduate qualification in clinical pharmacy
  • Post graduate qualification in psychiatric pharmacy or willing to work towards
  • Evidence of Continuous Professional Development
  • Postgraduate Pharmacist independent/supplementary prescribing practice certificate
  • Attended Leadership and Management training or have (or studying for) formal Leadership qualification (e.g. NHS LA, CPPE)
  • Associate or Full Credentialed Member of College of Mental Health Pharmacists
  • Completion of CPPE Mental Health module
Experience
  • Experience of mental health pharmacy or Clinical experience in a variety of settings
  • Supervised or managed others /Project management experience
  • Working as a lone pharmacist or pharmacy professional in a team or clinical setting
Knowledge
  • Confident and articulate communicator both written and verbal
  • Ability to work independently and as a member of a team
  • Understanding of the risk issues associated with medicines
  • Understanding of health service issues in primary and secondary care
  • Critical appraisal skills
  • To use the Internet to provide useful information and search bibliographic databases
