Band 5 District Community Nurse

Fast Track Compliance Team Support Healthcare are currently recruiting District Community Registered Nurses to provide community and hospital support for our clients in Liverpool. Location

Liverpool Hours

Full-time and part-time hours available Pay rate

Up to £30 an hour Main Duties/ Responsibilities Deliver clinical care to a defined population. Provide care to patients who have recently been discharged from hospital and require additional support. Recognize, assess, and refer patients presenting with physical or mental health needs. Perform comprehensive assessments of housebound patients nursing needs. To work effectively and collaboratively within the team to ensure effective and safe care is delivered to assist timely hospital discharge and prevent re-admission. Collect and report information, maintaining accurate patient records in line with NMC recommendations and Trust policy. Communicate effectively with patients and carers, recognizing the need for alternative methods of communication to overcome different levels of understanding, cultural backgrounds and preferred ways of communicating. Essential requirements Be a NMC Registered Nurse Must hold a full UK driving license with access to a car 6
