The successful candidate would need to demonstrate high level interpersonal and communication skills to provide and receive complex and often highly sensitive information. Be interested in using strengths based, solution focused strategies and motivational interviewing skills to recruit and engage eligible, hard-to-reach pregnant young women and enable families to develop behaviour change strategies.

The Family Nurse would be expected to develop therapeutic relationships with clients and work intensively within often challenging family situations to achieve the expected outcomes of the FNP Programme. This is a demanding specialist role requiring high levels of professional skills and practice and the ability to work well individually, as part of a team and in collaboration with local statutory and mainstream services. It is essential that candidates are able to maintain a high level of record keeping and are able to undertake data analysis.

Main duties of the job

Undertake home visits in accordance with FNP model

Use high level interpersonal and communication skills

Utilise specialist knowledge acquired through the FNP learning programme to deliver the FNP programme effectively

Work effectively in potentially highly distressing or highly emotional circumstances

Develop therapeutic relationships

Use professional and clinical judgement to undertake detailed assessments of clients and their families

Actively engage and work with fathers and other members of the extended family to achieve positive outcomes for children.

Apply knowledge and skills to a range of clinical and family situations

Work within the FNP model to initiate, plan and co-ordinate clients care in consultation and collaboration with other professionals and agencies involved in providing services to women and their families. This includes initiating CAF and taking lead professional role where required

Keep baby and young person in mind and work within local safeguarding policies as appropriate

Act on own interpretation of assessed clinical situation within the home setting

Contribute to the delivery of the HCP

Manage own workload by working independently within appropriate occupational guidelines, referring to FNP Lead where appropriate

Act on own interpretation of assessed clinical situation within the home setting. Able to refer client onto other specialist services as required on own authority

Complete full, accurate and contemporaneous records

About us

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by July 2024.

Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.

The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.

Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on 1st July 2024. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

Qualifications

Essential


  • Nursing or Midwifery qualification and registered with the NMC
  • Degree level or equivalent professional qualification or experience
  • Training, experience and short courses equivalent to Masters level
  • Willingness to undertake specialist post graduate training and to be assessed on competence in clinical field following training

Desirable


  • Knowledge of attachment theory, self-efficacy theory, human ecology theory and behavioural change theory
  • Parental/mental health knowledge
  • Understand the meaning of improving working lives
  • Motivational Interviewing knowledge
  • Counselling
