An exciting opportunity has arisen for a Community Learning Disability Nurse to join a highly experienced multi-professional health care team working in the Cornwall Adult Community Team - West.

The post-holder will be based within the Learning Disability Team in Pool, Redruth.

The clinical work is multi-disciplinary involving assessment, formulation and provision of therapeutic interventions, including positive behavioural approaches as part of the multi-disciplinary care planning and delivery.

We are actively engaged in the development of the service we provide with our community clients and your thoughts, ideas would be welcomed.

Main duties of the job

As a professional in the Health Community team, you will be responsible for coordinating the care the person receives from the Learning Disability service, in line with agreed Care Processes and Pathways. You will work actively with the individual and their families and/or carers in assessing the person's health needs, developing a care plan with them, implementing, and evaluating interventions, and reviewing the expected health outcomes.

  • Professional Registration with Nursing and Midwifery Council - RNLD/RMN/RN
  • Completion of, or willingness to complete, relevant preceptee programme if appropriate
  • Relevant clinical experience in a community or health care setting

Desirable


  • To act as a mentor for learners. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required.
