To work as part of a multidisciplinary team to provide health care, and support under the direction and supervision of registered staff to patients living in the community. This includes assisting in the delivery of interventions, treating and discharging patients and reporting all changes in patient status to senior staff.

Main duties of the job
  • To respect and acknowledge professionals skills and boundaries, taking opportunities to develop self-knowledge, contributing to effective team work.
  • To deliver holistic care when undertaking interventions to include pressure ulcer prevention advice and care, dementia awareness, observing nutrition and hydration status, falls risks, pain awareness and applying infection control standards.
  • To deliver clinical interventions delegated by registrant, as appropriate to role and competency.
  • To actively participate as a member of the multi-disciplinary team including contributing to meetings.
  • To actively participate in the induction / orientation of new staff and students.

Job responsibilities
  • To undertake non-complex clinical procedures under the guidance of senior team members e.g.: venepuncture, patient specific sub-cutaneous injections ,sample collection, blood pressure monitoring, medications, wound care, catheter care and other tasks according to training and competencies.
  • To recognise their responsibility and accountability in accepting the delegated tasks
  • Empowering patients to regain their health and independence in collaboration with other members of the Multi professional Team.
  • Liaising with clients, colleagues, family and other professionals in ensuring integrated quality care is provided.
  • To keep accurate, timely records according to Trust policy.
  • To escalate and report patient progress/ status on a shift by shift basis to senior team members.
  • To demonstrate flexibility and adaptability working across community teams according to service needs.
  • To commit to undertaking training and education to progress clinical skills as indicated by service development needs.
  • To respond effectively and actively to complaints and escalate appropriately following departmental policies.
  • To assist registered staff in care and treatment and participate in the review process.
  • To promote equality for all, including the maintenance of confidentiality, supporting individual customs, values and spiritual beliefs and ensuring that anti discriminatory practices are demonstrated.
  • To support carers in their role of carer. This may include communication and liaison with other agencies, including partnership agencies.
  • To provide practical / personal support for people identified as having complex needs e.g. self-neglect, at risk of abuse, challenging behaviour.
  • To support clients with their personal and emotional care needs, whilst respecting their dignity and maximising their independence.
  • To actively participate and contribute to the supervision process.
  • To exercise good personal time management, punctuality and consistent reliable attendance
  • As part of the team, incorporate up-to-date techniques and evidence based practice into your work.
  • To participate in audit and evaluation of the service as requested by your supervisor.
  • To support new or less experienced employees by demonstrating duties undertaken.
Person Specification Qualifications
  • Good standard of education
  • NVQ level III in Health Care or equivalent experience
  • Care Certificate completed or willing to work towards
Experience
  • Experience of working in a caring environment,
  • Experience of working with adults and/or experience of working in community or home care services
  • Basic understanding of statutory health and social services
  • Basic numeracy and ability to write simple reports
Skills
  • Good verbal communication and interpersonal skills
  • Ability to work as an effective member of a team
  • Ability to visit patients in the community unsupervised
  • Ability to deal with a wide range of people at all levels, including clients, carers, health and social care colleagues and other professionals
  • Competent ability to assist in the treatments of a wide range of conditions
  • Good use of reasoning skills
  • Awareness of holistic patient needs
  • Ability to work independently and demonstrate initiative
  • Willingness to undertake job related training and multi-disciplinary competencies
  • Able to prioritise and plan own workload
  • Familiarity with current NHS and social care policies
  • Ability to understand work related boundaries and refer matters of importance to a senior colleague
  • Ability to follow detailed instruction
  • Ability to document efficiently and effectively
