A unique opportunity has arisen in the Pharmacy Department at Royal Brompton and Harefield hospital. The Anticoagulation Clinical Nurse Specialist post is a dynamic role, which encompasses the management of the outpatient anticoagulation service at Harefield Hospital on a day to day basis with working hours of Monday-Friday, predominantly 9am to 5:30pm. The post-holder will work very closely and under the supervision from the Senior Pharmacist, Anticoagulation and Surgery. This post will be based at Harefield Hospital (with cross-site working at Royal Brompton Hospital) .

The ideal candidate will have experience of managing and prioritising care ideally within an Anticoagulation, Outpatient or Cardiology setting. The post offers opportunity for career development with the potential to undertake an independent prescribing course. Clinical duties will involve the smooth delivery of the anticoagulation service, ensuring the highest standard of patient care across our outpatient areas and supporting inpatient wards to deliver specialist care.

Main duties of the job

The post holder will be responsible for managing the outpatient anticoagulation service and providing specialist anticoagulation advice for patients that require warfarin dosing under the leadership of the Senior Pharmacist, Anticoagulation and Surgery and the Consultant Haematologist to ensure the safe and effective use of anticoagulation always working in a manner that promotes dignity through the adoption of person centred care principles.

The postholder will assist clinical areas with inpatients starting or continuing on oral anticoagulation where staff need advice and/or support.

The post holder will be involved in audit, research and teaching in anticoagulation and the prevention of venous thromboembolism.

The post holder will contribute to the development of guidelines, procedures and protocols in anticoagulation and venous thromboembolism.

Job responsibilities

The main duties and responsibilities of the Anticoagulation Clinical Nurse Specialist includes supporting the pharmacy surgery team and multidisciplinary workforce in contributing to the delivery of high standards of clinical leadership and excellent standard of patient care.

To be responsible for developing and managing the outpatient anticoagulation service across Royal Brompton and Harefield hospitals to ensure consistent clinical cover. This will include regular review of the service and the organisation and implementation of any changes.

To work under the Trust protocols and guidelines for anticoagulation and support clinical areas in adhering to areas of compliance and governance.

Qualifications

Essential


  • 1st level registration with NMC

Desirable


  • Degree level education
  • Teaching and assessing qualification (ENB 998 or equivalent)
  • Specialist cardiothoracic nursing qualification (ENB 249/254 or equivalent)
  • A registered independent prescriber

Experience

Essential


  • substantive post registration experience, which should be in cardiac nursing, at Grade 6 or above
  • Experience in teaching both staff and patients

Desirable


  • Experience in anticoagulation treatment
  • Involvement in writing guidelines/protocols/ standard operating procedures
  • Involvement in clinical audit
  • Experience in managing change
