Job Title: Adults Social Worker - Community Mental Health Team Location: Rhondda Cynon Taf
Randstad are working closely with RCT to recruit a dedicated and compassionate Adults Social Worker to join our dynamic Community Mental Health Team. This role offers an opportunity to make a real difference in the lives of adults facing mental health challenges. You will work collaboratively with service users, families, and multi-disciplinary teams to deliver tailored support and interventions aimed at promoting independence, recovery, and well-being.
Assessment and Support Planning: Conduct holistic assessments of service users with mental health needs, developing individualized care and support plans.
Risk Management: Identify and manage risks, ensuring safety and well-being of service users while balancing the principles of empowerment and autonomy.
Multi-disciplinary Collaboration: Work closely with healthcare professionals, housing services, and other agencies to ensure integrated, person-centered care.
Safeguarding: Recognize and respond to safeguarding concerns, taking action in accordance with local authority policies and procedures.
Support service users to access the services and resources they need, advocating on their behalf to promote inclusion and recovery.
Case Management: Maintain accurate and timely records, manage caseloads effectively, and ensure that care plans are regularly reviewed and updated.
Provide immediate support and intervention in times of mental health crisis, ensuring appropriate services and care are in place.
Qualifications: A degree in Social Work (or equivalent) and registration with Social Work England.
Experience: Proven experience in adult social work, ideally within a mental health setting or community-based services.
